TeleNurse Network is developing a Web-Based Application that permits users to connect with specialty nurses and advanced nurse practitioners via a HIPPA compliant audio/video chat, inclusive of EMR, and E-Prescribe. TeleNurse Network aims to improve access to care by using Telehealth technologies for remote diagnosis, education of chronic disease, and patient monitoring. This strategy is an optimal health solution that ensures “specialty care” which normally is not accessible due to high cost or barriers in transportation.

TeleNurse Network is geared towards increasing access to services for minority, low-income communities addressing key challenges:

1.) fragmentation in preventative care,
2.) discontinuity in care post discharge from ambulatory care centers and hospitals,
3.) knowledge barriers/deficit on management of chronic conditions, &
4.) limited patient engagement post hospitalizations.
